- Service and Information in Your Language: to request an interpreter for any communication regarding your child's treatment.
Respectful and Courteous Treatment
- Be treated with respect, dignity, and courtesy.
- Receive care that is considerate of your personal values and belief system.
- Be free from retaliation or force of any kind when making decisions about your care.
Privacy and Confidentiality
- Have a private relationship with your provider and have your medical record kept confidential.

Choice and Involvement in Your Care
- Request an accurate and current set of professional credentials of the practitioner working with you.
- Receive information about ABC's services and the practitioners involved in your child's care.
- In urgent situations, be given enough information about your child's treatment to make an informed decision regarding that treatment.
- Have a candid discussion with your treating professionals about appropriate or medically necessary treatment options for your child's condition, regardless of cost or benefit coverage.
- Participate in treatment planning (if over the age of 12) and have family members participate in such planning.
- Receive individualized treatment, including:
- Adequate and humane services regardless of the source(s) of financial support.
